Form and Document Information

You will need these to file your taxes. If you are Married Filing Jointly, you will need these for your spouse as well.

  • Driver’s License or State ID
  • Social Security Card
  • Proof of Residence

These are the common tax forms you will need to bring with you to file your taxes.

  • W-2 Forms: Income from jobs
  • 1099 Forms (-INT, -DIV, -B, etc.): Investment income
  • 1099-G: Income from state and local income tax refunds and/or unemployment
  • Taxable alimony received
  • 1099-R 8606 Forms: IRA/pension distributions
  • SSA-1099: Social Security benefits
  • 1099-C: Income from sales of property—original cost and cost of improvements, escrow closing statement, cancelled debt information
  • Business or farming income—profit/loss statement, capital equipment information
  • If you use your home for business—home size, office size, home expenses, office expenses.
  • Rental property income/expense—profit/Loss statement, rental property suspended loss information
  • Other miscellaneous income—jury duty, gambling winnings, Medical Savings Account (MSA), scholarships, etc.

  • Advance Child Tax Credit payment
  • Child care costs—provider’s name, address, tax id, and amount paid
  • 1098: Home mortgage interest and points paid
  • Real estate taxes paid (on properties that are not your primary home)
  • 1098-T: Education expenses
  • Student loan interest paid
  • Charitable donations—cash amounts and value of donated property, miles driven, and out-of-pocket expenses
  • Medical and dental expenses
  • Casualty and theft losses: Amount of damage and insurance reimbursements
  • Other miscellaneous tax deductions—union dues, out-of-pocket necessary employment expenses, adoption costs, investment expenses

These are much less common, but needed for some people with specific careers or expenses.

  • IRA contributions
  • Energy credits
  • Self-employed health insurance payments
  • Self-employed pension plans
  • Educator expenses
  • Personal property taxes
  • Estimated tax payment made during the year, prior year refund applied to current year
  • Foreign bank account information—location, name of bank, account number, peak value of account during the year

These are the forms that you may need to use for your taxes.

Form 1040 – US Individual Income Tax Return
  • Annual income tax return filed by citizens or residents of the United States.
  • This is the most common tax form, and often the only one you need.
Form W-9 – Request for Taxpayer Identification Number (TIN) and Certification
  • Used to request a taxpayer identification number (TIN) for reporting on an information return the amount paid.
 
Form W-4 – Employee’s Withholding Allowance Certificate
  • This is so that your employer can withhold the correct federal income tax from your pay.
 
Form 9465 – Installment Agreement Request
  • Request a monthly installment plan if you cannot pay the full amount you owe shown on your tax return.
 
Form W-7 – Application for IRS Individual Taxpayer Identification Number
  • Get or renew an individual taxpayer identification number (ITIN) for federal tax purposes if you are not eligible for a social security number.
 
Form 4506-T – Request for Transcript of Tax Return
  • Use this form to order a transcript or other return information free of charge, or designate a third party to receive the information.
Form 8822 – Change of Address
  • For Individual, Gift, Estate, or Generation-Skipping Transfer Tax Returns
 
Form 1040-ES – Estimated Tax for Individuals
  • Use Form 1040-ES to figure and pay your estimated tax.
